- The following Forms need to be submitted by applicant for registration and issuance of Death certificate in case of Home death:-
- Death registration certificate request form.
- Form No.8.
- Death report Form.
- Form No. 4A.
- Document required for Death Registration and issuance of Death certificate within 1 year in case of Home death:-
- Doctor death certificate of cause of death.
- Copy of Cremation Pass.
- Ration card/Aadhaar card.
- Tax Receipt.
- Bank Pass Book Xerox or I.D. Proof of the deceased person.
- Ward Member certificate.
- Document required for Death Registration and issuance of non availability Death certificate after 1 year in case of Home death:-
- Doctor death certificate of cause of death.
- Copy of Cremation Pass.
- Ration card/Aadhaar card.
- Tax Receipt.
- Bank Pass Book Xerox or I.D. Proof of the deceased person.
- Ward Member certificate.
- Affidavit on 100 Rs. Stamp paper.

- Hospital Death.

- Death registration certificate request form.

- Postmortem report.
- Document required for Death Registration and issuance of non availability Death certificate after 1 year in case of Accident/Suicide:-
- Doctor death certificate of cause of death.
- Copy of Cremation Pass.
- Ration card/Aadhaar card.
- Tax Pawati.
- Bank Pass Book Xerox or I.D. Proof of the deceased person.
- Ward Member certificate.
- Affidavit on 100 Rs. Stamp paper.
